Clinical Evaluation of Wheezing Children


Abstract
The author evaluated clinical menifestations, laboratory test and allergic skin test in 109 preschool children who had a symptom of wheezing.
@ES The results were as follow:
@EN 1) In the age distribution at onset, below 1 year of age were 10 cases(9.1%), and above 1 year of age were 99 cases(90.9%), Male to female ratio was 2:1.
2) The cases of below 50% tile of body weight and height were 67(61.1%) and 92(84.4%) cases respectively.
3) We diagnosed acute bronchiolitis in 58 cases(53.2%) bronchial asthma in 18 cases(16.5%), persistent wheezing in 14 cases(12.9%). Others were laryngomalacia, foreign body aspiration, mediastinal mass and vascular ring.
4) The mot frequent season of attack was winter(38.9%). The other are autumn(28%), spring(19%) and summer(14%) in order. In the number of attack, the cases of 1 attack were 38 cases(38.9%) and those of above 7 attacks were 25 cases(22.9%), but
the
number of attack was evenely disributed.
5) In the evaluation of the eosinophil count in the peripheral blood and IgE level in the serum, above 400/mm3 of eosinophil was 28.4% and above 250 IU/mL of IgE level was 34.8%. We recommanded allergic skin test for the patients of increased
eosinophil count or IgE level, but this test was performed in 27 cases, of them, 18 cases(66.7%) had positive result.
6) On initial laboratory finding, mean erythrocyte sedimentation rate and white blood cell count were increased.
7) There was no significant difference between staphylococcus aureus and other groups in duration of fever, duration of joint symptom, duration of admission. But in staphylococcus aureus group, the duration of symptom was more longer.
8) Most (36 cases) were treated by drainge and antibiotics, 10 cases by aspiration and antibiotics 6 cases by only antibiotics.
9) The sequelae developed on 12 of 52 cases(21.2%), but there were no significant difference between groups.
After the discovery of antibiotics, the mortality rate in the acute stage of the childhood pyogenic arthritis was markedly decreased. However many complication was resulted from inadequate such as antibiotics abuse, late treatment and
inappropriate
antibiotics.
Therefore, early diagnosis and early therapy is necessary for complete recovery without complication in pyogenic arthritis of
